FDA approves Revolution Medicines' Rasonque pill for metastatic pancreatic cancer

The US Food and Drug Administration approved daraxonrasib, marketed as Rasonque, after clinical trials showed the once-daily pill extended median survival to 13.2 months for patients with metastatic pancreatic cancer.

The US Food and Drug Administration granted expedited approval on Wednesday for daraxonrasib, marketed under the brand name Rasonque by California-based Revolution Medicines. The oral medication is indicated for adult patients with metastatic pancreatic adenocarcinoma who have undergone prior systemic therapy or cannot tolerate multi-drug combination chemotherapy regimens. Regulatory officials cleared the treatment 6.5 months ahead of the target user-fee date by utilizing several expedited pathways, including Breakthrough Therapy designation, Orphan Drug status, Priority Review, and the Commissioner's National Priority Voucher program. The agency noted that pancreatic adenocarcinoma represents between 90% and 95% of all pancreatic cancer diagnoses in the United States. Acting FDA Commissioner Kyle Diamantas emphasized the agency's commitment to expediting treatments for difficult conditions.

Clinical trial results

The regulatory approval was supported by data from a company-funded, randomized, open-label, multicenter clinical study involving 500 adults with previously treated metastatic pancreatic cancer. Patients receiving Rasonque demonstrated a median overall survival of 13.2 months, compared to 6.7 months for participants who received standard chemotherapy protocols. Investigators reported that the once-daily pill produced fewer severe adverse side effects than traditional multi-agent cytotoxic regimens, while some clinical participants survived for multiple years during extended tracking. The therapeutic mechanism focuses on inhibiting multiple forms of the RAS protein family, which regulates cell growth and harbors driver mutations in more than 90% of pancreatic adenocarcinoma cases.

Pancreatic adenocarcinoma remains one of the most lethal oncological conditions, largely because early detection is rare before malignant cells disseminate to surrounding organs like the liver. According to data cited from the National Cancer Institute, pancreatic cancer constitutes approximately 3.2% of all annual US cancer diagnoses, yet it accounts for a disproportionate share of cancer mortality. The American Cancer Society projects approximately 67,000 new US diagnoses in 2026, with annual deaths expected to surpass 52,000. Across all patient stages, the five-year overall survival rate is 13%. Outside the United States, the malignancy impacts more than 1,000 people annually in Portugal, where late-stage clinical presentations similarly limit curative surgical options.

Expanded access and commercial rollout

Public awareness of the treatment expanded earlier in 2026 after former US Senator Ben Sasse of Nebraska shared his personal treatment experience on CBS's 60 Minutes program, noting pain reduction during therapy. A sharp rise in public and patient inquiries prompted the FDA to authorize an expanded access protocol in May, allowing qualifying individuals to obtain the medication prior to full regulatory authorization. Individual patient experiences, including those of former movie studio executive Jay Williams whose cancer spread to his liver following surgery and chemotherapy, reflect the progression patterns common among advanced cases. Revolution Medicines has not yet disclosed commercial pricing tiers or an exact retail launch timetable. Oncologists noted that dozens of experimental compounds targeting RAS mutations remain in active development, offering further avenues for combination therapies.
